Use the [+] button to add the second specimen, and the [...] button next to taxon to add the determination, in each case.

I'm totally confused now. After looking at this one, why was the Illinois Natural History Survey (ILLS) removed as a Herbarium?? Since they have a label on the page and they had someone do a determination for the species I'm not getting why it was removed. ILLS is one of 3 Herbariums located at Evers Lab per Google. Could you please explain why they are not shown as a Herbarium? Thanks so much!

This sheet is clearly from the FSU herbarium as stated top centre of the main label and in the top right corner of the sheet. The reference to Illinois is on the determination slip and refers to Levin's address in 1995 when he identified the specimen(s). Although he may have been working in that herbarium at the time there is no evidence that the sheet was retained there. Please check through your other sheets for similar references!
