After much staring, I came up with:
Hailey Wood VC 33 (East Glos); fide A Ley (in my ignorance I would just enter A Ley as second collector - green +); var incisum (could go in notes - optonal I think).

tip from Mossysal:
RE your comments to Jupiter, http://herbariaunited.org/forum/viewtopic.php?t=5058, it seems that Ley was busy in Shropshire that day so not the second collector. I personally would put him in as determiner in this case - do you not think so?

I think this a transcribed label, and the site may be an error for Hailes Wood SP0430, NE of Wichcombe, whence Riddelsdell has several specimens same date
