Welcome to H@H I noticed your post on this specimen and have taken the liberty of reviewing the sheet, hope you don't mind.
Very many of Gloucestershire sheets have no collector and I would assume in this case that the specimen was collected or sent to this Herbaria, so Ex Herb would be Gloucester museum or who-ever it was submitted too. There is no need to name the Ex Herb in these cases.
The details of where collected if present, go in the box above "notes" OS grid reference is automatically inserted into the box above this.
Unless the place name is not in the data base, in which case, you can find it by clicking on the small buttons to the right of the grid ref box, or using Google Earth.
The accession number is mostly in the top right on these sheets.
After a while you will get to recognize collectors 'styles'.
