The collector is Hayes or Bloxam.

The locality is not 'Calverton' but CLAVERTON (Bath); I can't read the other.

handwriting & printed labels are Hayes/Bloxam's. They certainly collected at Claverdon Warks in this month, so despite the ambiguous writing I think this must be the site. Honiley in Warks is also a favourite place of theirs.
