Evidently a collation of two gatherings (because of the flowers and fruit, both on leafy twigs). I would therefore interpret the notes as: "Easton Essex May 1864; Wood Green Aug. 1869". You can use the buttons on the form to add "extras" to cope with this sort of thing.
