The record you've created looks fine. P.C.H as collector refers to Peter C Holland - it's probably better to enter the name in full, so that records can be easily linked in the database.

The Dandy number is a reference to J.E.Dandy's List of British Vascular Plants - so just an obsolete taxon id code - there's no need to record it. Similarly CTW refers to Clapham, A.R., Tutin, T.G. & Warburg, E.F., Flora of the British Isles. The other numbering scheme you may come across on many sheets is LC - 'London Catalogue of British Plants'. Herbarium sheet labels often get covered in codes which, with few exceptions, are no-longer useful.
