This collector might be Rev J H Bloom (1860 - 1944), Rector of Whitchurch, Warwicks, antiquarian, (http://en.wikipedia.org/wiki/James_Harvey_Bloom).

His 1902-09 entomological notebook relating to Whitchurch, Warwicks, is at the Natural History Museum; he wrote "Shakespeare's Garden" (1903) but I cannot find any other botanical associations.

I see the sheet suggested by Mossysal is identical in layout and handwriting, despite the menu not bringing up anyone called Bloom when I tried ... I am editing them both as J H Bloom (and entering the usual caveat)
