I've just released a new revision of the documentation form. There are many minor tweaks and bug-fixes but the significant changes are:

Grid references
It's now possible to specify grid references. This is useful to record is the sheet label specifies a grid ref. It's not necessary to look up grid references if none is specified on the sheet, though if you can do this, and are confident you have found the correct place then please feel free to add a grid reference, as this will make the record more useful. Most place-names in the online dictionary already have a 2-figure grid ref that will be used by default if no reference is specified.

For Britain (but not yet for Ireland) grid references are cross-referenced against county, so this can be a good way to verify that you have the correct vice-county.

Multiple dates
It's now possible to separately record multiple dates or date ranges. To do this click on the green '+' to the right of the date field to add more entries.

Preferred taxon name
You can now explicitly state the current preferred taxon name, if there are multiple determinations. To do this tick the 'preferred' box of the taxon field (this only appears as an option if there are multiple names). Previously the top name on the list was treated as the current name.

Please let me know if you encounter any problems, or have suggestions for further changes. (the new version has been tested with many browser/operating-system combinations, so I'm not anticipating any new bugs...)
