I have edited this sheet to include the collector's name and the host species (both as labelled) from which I have inferred the taxon and vice-county.

From http://herbariaunited.org/specimen/236943/ one might further infer that the locality was Richmond, Guernsey, and the plant O. minor var. maritima collected on 13/8/1896 but that might be seen as stretching the detective process beyond its legitimate extent.
