Sorry about the confusion over this. The new 'TYPE' button at the top-left of the form can be used to add an extra field to the form, which allows the type status of the specimen to be noted.

I'm having second thoughts about doing it this way, as really the type field should be associated with the determination, rather than as a separate entry.
