I would interpret it as at least two specimens: Ellesmere, Shropshire and Llandrindod, (Radnorshire?). As they have separate dates, they're probably better listed as two records, rather than two sites for the same one. The Cornwall etc. is a bizarre addition, which strikes me as pretty useless, and not worth documenting.
