Can't make sense of the middle 3 lines of script: "Form millosa (???)?? Croftbanks Chorley? ?????????shire" or the collecter's initials. Any ideas?

I think part of the place name reads 'Cradley', Birmingham, but I can't decipher the 'croft' part above.
The collector's initials are R F T.

Viola riviniana f. villosa, (Neum., W. & M.) -- the author matches that given in the London Catalogue
