The writing looks a lot like the script on many of the Hayes or Bloxham donated specimens - can't recall seeing any with a collector's signature either....That said just tried searching for them and unable to get the database to accept a query for "Hayes or Bloxham" ....

It does look like a chopped up Hayes/Bloxam sheet.

I don't know what the background to the 'Hayes or Bloxam' sheets is - a few have collector names - in those cases they are often sheets distributed through the exchange club (initials BEC or LBEC (London Botanical Exchange Club) handwritten on the sheet, generally in the middle-top).
