The way you've documented this sheet looks fine. There's no need to record the vice-county number in the notes field as this already gets included with the 'region' field.

The other minor point (which only applies to sheets from Bolton Musuem) would be to include the accession number stamped on the sheet, in this case 30-07-25574 in the corresponding boxes on the form. This is needed so that when records from the project are merged incorporated into the museum's database there is an unambiguous link between each record and a physical specimen.
