Bearing in mind Baker's somewhat variable spelling - then Cleaves might be a possibility ??

I see Liffen got there first. Cleaves is at SE498836
The circumstantial evidence prompts me to say this is probably ex herb G S West and very possibly also ex herb M M Atwood.
