The acc no is hidden at bottom right; the initials are Mrs F S Bennet. But the site is a mystery to me, I couldn't find a match on Streetmap. F S B's sheets are usually anonymous, and often collected by other people as is evident from some early dates; but this style of printed sheets (which somewhat resemble Hayes/Bloxam's) are always hers. She mostly collected in Cheshire.
