Hi there. Heath's collections are numerous and familiar to the point of excess! The layout of the sheet is typical, showing from experience that it can be edited as collector: D M Heath; ex herb: D M Heath; as so often there are specimens from more than one site (Isle of Wight is the county in both cases; 'landslip' I have never located so I put in the site detail box); entering 2 separate specimens is best done by clicking the green + above the other boxes marked 'specimen' - good luck!
