Surely it's coll. Moyle Rogers with E S Salmon's det slip.

Miles's det slip is uncompleted so should be ignored.

The pencilled det is in a naggingly familiar hand. Whose?

At first, I assumed these pencilled identifications were W C R Watson's, but the handwriting is not really the same. Although I'm not seriously proposing it, the most similarity I saw, was with E F Warburg's
