Is this
the correct way to enter "site" "notes" and "taxon"? Could original taxon have
been included?

As a newcomer I'd be interested in reading a reply from someone more experienced than me.
I think you've entered the location as I would have done.
I initially added notes of all the text on the sheet but I notice that other sheets rarely have them so I don't do it any more.
It looks as though you can add other taxon names from the sheet but other sheets don't often have them added.

Yes; overall this spmn has been adequately documented.

Some users of Herbaria@Home, e.g. Vc Recorders, are satisified with nothing more than the Vc number which serves to flag the spmn for them; but most viewers of documented sheets welcome all pertinent information. In that connection nothing can be more subversive of the scientific value of this project than careless and inaccurate documentation.

There are quite adequate guidelines at http://herbariaunited.org/wiki/How_to_document_a_herbarium_sheet and at "show extended help text" on the documentation form. I wonder if anyone reads these.

You can't do any worse than just following the guidelines and asking for help when necessary.
