N.B. reporting of the edit history is currently fairly unclear and misleading. Most edits made to specimens appear as a pair of 'add' and 'delete' entries, which may not be together in the list. There are also often 'minor' edits, which are made automatically (rather than due to user activity), for example to merge synonym names.

What does the number in pencil mean? Does it need to be included?

The W Wats after the taxon name is the botanical authority for the name and doesn't have to be included.
The "Yes B A Miles" I think indicates that the ID has been confirmed by B A Miles. Click on the ... symbol after the taxon name to get another menu and choose 'confirmed by'.
I'm not sure about the number, very few other specimens which have been documented seem to show them so I leave them out.
I'm a beginner myself, so a more experienced contributor might want to correct me.
