I looked up the grid ref ST 672311, it's Higher Shepton near Shepton Montague, Somerset, so there is no collector's name. The script looks distinctive though! The date looks like 10/5/1966, but the era of the script is more like 1905...... I made a search of the specimens from Shepton Mallet etc but found nothing similar
