The collector wrote only his initials on the sheet and they're almost illegible. They just might be 'SEC', so the collector could be the same Chandler as shown under 'herbarium'.

The initials match those of S E Chandler

Probably the three distinct taxa on this sheet should be documented as separate entries (you can add extra copies of the documentation form by clicking the green cross near the top-left of the form).
