Does the lack of collector and date mean this shd not be listed? Or does anyone know whether the B.A. Miles det. means that it was coll. him in 1963?

You don't need to put Anon if there is no collector, just leave it blank. The determiner goes in the top box [species name] by clicking the '...' I have edited that bit for you, and determiner is no unlikely to be the collector [at least in this case].
